What GLP-1 Medications Really Cost

Sticker prices, what people actually pay, and the single biggest factor that decides your monthly cost.

GLP-1 pricing is genuinely confusing — list prices, insurance games, savings cards, and compounded alternatives all collide. Here's the honest map.

Brand-name list prices (without insurance)

Brand-name GLP-1 medications carry high list prices in the U.S. Without insurance coverage, monthly costs generally land in this range:

MedicationMoleculeApprox. monthly list price*
Wegovy (injection)Semaglutide~$1,300+
OzempicSemaglutide~$1,000+
ZepboundTirzepatide~$1,000–$1,300+
MounjaroTirzepatide~$1,000–$1,300+
Wegovy (oral pill)SemaglutideStarting ~$149 (intro pricing)

*Approximate cash/list prices; actual prices change often and vary by pharmacy, dose, manufacturer savings programs, and region. Always confirm current pricing directly.

Manufacturer self-pay options exist

Both Novo Nordisk and Eli Lilly now offer direct cash-pay programs (for example, vial-based self-pay pricing) that can be lower than the traditional list price. If you want a brand-name product, ask about the manufacturer's own self-pay pricing.

Compounded semaglutide & tirzepatide

Compounded versions — prepared by compounding pharmacies rather than the brand manufacturer — are usually dramatically cheaper, often a few hundred dollars per month or less through telehealth platforms. The tradeoff: they are not FDA-approved, and 2026 rules restrict when they can be compounded. Understand that tradeoff first in our brand vs compounded guide.

The factor that decides your monthly cost

With telehealth compounded programs, the length of your plan usually matters more than anything else:

Plan lengthTypical cost per monthBest for
Month-to-monthHighestTrying it briefly (but you pay a premium)
3-monthLowerA real trial run
6- or 12-monthLowest per monthCommitting to results (best value)

Why the longer plan usually wins

GLP-1 results build gradually as you titrate up over months — stopping early often means stopping before you'd see the full benefit. Since longer plans also carry the lowest monthly price, the annual plan tends to be both the most effective and the most affordable choice for people who are committed.
